In consideration of accepting this entry in the 2010 Kids Triathlon, I the undersigned parent/guardian, intending to be legally bound, do hereby, for myself, my heirs, executors, administrators and assigns waive and release any and all rights and claims for damages I may have against Habitat for Humanity of Greater Pittsburgh and /or any and all sponsors of this event, their officers, directors, agents, employees and any representative thereof, their successors and assigns, for any and all injuries suffered by my child/ward, of any nature whatsoever in this event. I understand that a triathlon is a potentially hazardous activity and I hereby certify that my child/ward is medically able and physically fit to participate in this event.

I, hereby, grant full permission to Habitat for Humanity of Greater Pittsburgh and any other sponsors of this public event full permission to use my child/ward’s name, likeness and voice as well as any photographs, videotapes, motion pictures, recording or any other record of this event in which my child/ward may appear for any legitimate purposes including television broadcasting of this event, the reuse of any media of this broadcast and in advertising and promotion for such broadcast and reuse.
